Care Coordination
Effective two-way communication between primary care and dental clinicians is critical. Patients who are particularly in need of care coordination include those who are:
- Medically complex
- Taking anticoagulants
- Require antibiotic prophylaxis
Primary care clinicians should:
- Actively address oral issues and educate patients about oral care.
- Provide dental consultants with written diagnosis and treatment recommendations.
- Consider using a standardized communication form.
